Trillium Health NY Support Services Coordinator- Nutrition/Education in Rochester, New York

Purpose: Develop nutrition health education plans and implement group and one-on-one nutrition education and access to foods that support improved health outcomes for individuals with chronic food related illnesses within the nine-county Finger Lakes Region. Participate in and coordinate efforts with all service areas contributing to client’s Comprehensive Care Plan for a person-centered and collaborative approach.

    1 . Nutrition Education

  • Develop Nutrition Education curriculum that aligns with grant or funding standards and with best practices in the field, with Registered Dietician support and/or oversight as needed.

  • Screen individuals interested in Nutrition services.

  • Schedule, plan, and present educational programming to individuals and groups within the service area as it relates to nutrition, in collaboration with other providers to meet goals and outcomes in the Comprehensive Care Plan.

  • Maintains client records that document attendance, participation, progress accomplishments, referrals and additional needs. Document all interactions in electronic health record and in collaboration with support staff, ensure required data entry is completed accurately and timely.

  • Provides oversight for all aspects of service delivery and assures client support and engagement within service guidelines, for successful outcomes.

  • Network with internal and external medical providers, registered dieticians, and care managers, and all other service areas to coordinate service delivery, awareness of services and solicit referrals.

  • Engage in required case conferences, assessments and planning every 180 days during client’s enrollment in program with collaborating care providers.

  • Work with clients and their care teams to achieve health related progress/outcomes, including building client’s independence over time, and improving self-management skills.

  • Provide presentations at meetings, design flyers, facilitate individual meetings, and conduct phone calls.

  • Coordinate room/space availability for assigned events.

  • Travel to rural areas (if needed) to provide program services.

    1. Community Coordination
  • Work with team members to facilitate referrals for clients that will enable them to access needed services such as food, nutrition, and financial resources (food stamps, WIC), case management services, and other needed community services.

  • Track participants’ referral outcomes to ascertain usage and completion of referrals within client record.

  • Establish connections and communicates frequently with medical nutrition programs/practitioners and Registered Dietitians. Makes referrals as necessary and appropriate.

  • Provide outreach activities and education opportunities within the community.

  • Increase individuals’ awareness of and access to local health and human service care system for the purpose of leveraging community resources which include: other nutrition programs, food meal and financial resources, primary care, care management, mental health, transportation and substance use services.

    Qualifications

    Bachelor’s degree required, Master’s preferred in Nutrition or related field; minimum of 1 year experience working within HIV/AIDS field, or with other chronic illnesses; experience developing and facilitating nutrition health education; good communication and writing skills; understands importance of community collaboration. Must have a reliable vehicle and meet the agency driving policy requirements for this position, including a valid NYS driver’s license and proof of required automobile insurance.

    Physical Requirements

    While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
